MEETING AND COUNSELLING FOR COUPLES

Every couple seeking treatment for their subfertility issues does it for the hope of fulfilling their dreams of parenthood. The first meeting is significant and would involve a talk with the doctor regarding the clinical, psychosocial and sexual details, which is important for finding out the issues and charting out the treatment. Once these are available, the doctor would prescribe a baseline investigation.

Both the male and female partners are analysed based on a standard proforma which helps in the close analysis of issues that contribute to the subfertility. Factors such as the cause of subfertility, the duration of the issue and the age of the couples, especially that of female partner is important in deciding the line of treatment.   

With increasing age, more specifically beyond the age of 35 years, the ovarian reserve in women diminishes. The risk of chromosomal abnormalities is also known to increase with increasing age. The duration of infertility plays a crucial role in determining the mode of treatment.

In subfertility issues that are relatively new, simple methods of treatment are initiated while subfertility issues that have been experienced for long needs more aggressive treatment techniques for the desired results. In couples with long-standing subfertility issues, the probability of spontaneous conception is very less.

A regular menstrual cycle indicates regular ovulation in about 95% of women. It is important to identify the type of infertility. Identification of the issue as primary or secondary infertility helps in ascertaining the course of treatment. In women who have experienced previous cases of miscarriages, the period of gestation when the loss occurred will help in identifying the problem. Other abnormalities like endometriosis and pelvic inflammatory diseases can be found out from the medical history and symptoms in the patient.

The initial baseline investigations include a physical examination of the abdomen, pelvic area and breasts, along with an assessment of TSH, day two FSH, LH, prolactin and screening test for HCV, HIV and HBsAg. 

In men, an analysis of lifestyle, nature of the job, habits such as smoking or drinking, if any and sexual health history are done to identify issues. History of any trauma caused due to accidents or diseases to reproductive organs such as testes should be conveyed to the doctor. The screening tests for the male partner involves HCV, HBsAg, Haemoglobin and random blood sugar test along with semen analysis.

The counselling for couples seeking subfertility treatment involves easing their mind of the emotional turmoil. This would include a detailed interaction and building of a rapport between the doctor and the patient. The patient’s issues are analysed through baseline investigations and the outcomes are discussed with them. The treatment plan is decided based on the patient’s priorities and interests. Counselling helps in easing the worries and confusions of the patients through an open interaction with the doctor.
